Casomorphin and gliadorphin are opioid-like peptides formed when casein (dairy protein) and gluten (wheat, rye, barley) are not fully broken down during digestion. In healthy digestion, the enzyme DPP-IV cleaves these proteins into individual amino acids. When DPP-IV activity is low, overwhelmed by heavy dairy or gluten intake, or limited by inadequate zinc, the larger peptide fragments survive, cross a permeable gut lining into the bloodstream, and bind opioid receptors in the brain. The result can be food cravings and binge eating, mood changes, brain fog, inattention, and withdrawal-type irritability when the foods are removed.

The test measures both peptides in a first-morning urine specimen collected at home on an empty stomach, frozen, and shipped to DHA. It is intended for patients with suspected dairy or gluten driven symptoms, including binge eating and food addiction patterns, ADHD and attention problems, autism spectrum presentations, and treatment-resistant mood disorders.
