Overview

8-hydroxy-2′-deoxyguanosine (8-OHdG) is one of the most widely studied biomarkers of oxidative stress. When reactive oxygen species outpace the body’s antioxidant defenses, hydroxyl radicals preferentially attack guanine — the most oxidation-susceptible of the four DNA bases — producing 8-OHdG as a signature lesion.

Cellular repair enzymes excise the damaged base from the DNA strand. Because the excised nucleoside is not reincorporated and is cleared renally without further metabolism, urinary 8-OHdG offers a non-invasive, whole-body snapshot of the rate at which oxidative DNA damage is occurring and being repaired — not just a measure of antioxidant capacity in isolation.

Clinical Utility

8-OHdG helps quantify oxidative burden in patients where free radical stress is suspected to be driving or accelerating pathology. Elevated levels have been associated with:
